Job: P3.6 - MIDI output correctness in LV2 path

Task: Verify channel (1-16), velocity (1-127), durations (0.25-beat hits, 1-beat crash) in output.

Acceptance: LV2 capture matches the 240/960-tick expectations.

Reference: docs/PLAN.md P3.6

Status in plan: Partial (contract documented; LV2 capture not yet taken).

Automated verification extended in tests/lv2_smoke.cpp: first cycle now runs for 24000 frames and checks channel (10), note-on velocity 1–127, note 0–127, note-off velocity 0, frames in range, and presence of both note-on and note-off (7 events observed). Build + smoke test pass. Real host capture of the 240/960-tick durations remains under P3.10/P3.9.
